Identify the unit and condition
Capture the site, unit identifier, operator, opening hour meter and observed defects. The inspection should reflect actual conditions, not a pre-filled all-clear state.
Protect the status transition
An unsafe condition may keep equipment Not Active. Record the reason, start time and repair; authorization to return to service belongs to the designated accountable role and should be auditable.
